Housing Benefit and Council Tax Support childcare costs certificate

This form will ask you to provide

  • your details including National Insurance number
  • your Housing Benefit or Council Tax Support reference number (if known)
  • childcare provider contact details and address where the care is provided
  • the type of care that applies to you.

You need to provide a signed letter from the childcare provider stating:

  • their full name
  • their OFSTED or KCC registration number
  • their position
  • for all children under the age of 15, or 16 years old if the child is disabled, for whom they provide childcare:
    • the term-time weekly charge after government funding and meals have been deducted
    • the date the childcare started as this rate
    • the number of weeks per year that the child is cared for at the term-time rate
    • the weekly charge during school holidays (excluding meals)
    • the number of weeks per year at school holiday rate.

You can upload this letter to this form now or you can send them later but we won't process your application until we have received your proof. You must send your proof within the next two weeks otherwise we will cancel your application and you will need to reapply, if you wish to do so.
