This form is for local organisation to refer to the Inspire Fund for young people in the District to attend some extra-curricular activities.
We will ask you for the following:
- You contact information
- The young person’s details
- Reasons for referral
- How the young person feels
Please ensure you have discussed with the parents/guardians prior to completing the referral and they are happy for you to continue.
